Though you get it from food, vitamin B12 (or cobalamin) originally comes from bacteria. You get it almost entirely from animal foods: meat, poultry, fish, eggs, and dairy. 

To absorb B12 from food or from a pill, your body needs three things working together:

  • Stomach acid, which releases B12 from the protein it is bound to
  • Intrinsic factor, a protein your stomach lining makes that escorts B12 through your gut
  • A healthy small intestine, specifically the last section, where B12 is finally taken in

Should even one of these three steps break down, absorbing B12 from a tablet becomes difficult. This is the core of the B12 oral vs injection question. A pill is not a delivery system on its own. It relies on your digestion to finish the job.

An injection skips all three steps. The B12 goes into muscle tissue and moves into your blood without needing acid, intrinsic factor, or a working gut.

" Sublingual guarantees better absorption." It's a reasonable theory, though how well it works differs widely from one person to the next.

Oral B12. Tablets and capsules usually range from 50 mcg to 5,000 mcg. Because so little is absorbed, they are taken daily. The dose is often deliberately high, since most of it passes through and leaves the body. Changes tend to be gradual.

B12 injections. Injections commonly contain 1,000 mcg. Because absorption is far higher, they are given less often. Frequency depends on your situation. Correcting a significant deficiency often starts with weekly or biweekly injections until levels come up. Maintenance is usually monthly, though some people do better on a biweekly schedule.
